On Tuesday, July 16, 2002; Wednesday, July 17, 2002; and Thursday, July 18, 2002; Corrections Officer Gregory Blair used sick time for his scheduled duty days on First Watch. He submitted an Employee Leave Request report for this sick time on Friday, August 16, 2002, and a doctor's note for his sick time from Upper Valley Family Care dated Wednesday, August 14, 2002. Officer Blair wrote on his Employee Leave Request that his sick leave was a non-work related illness/injury due to "flu/illness in family." Officer Blair failed to submit his Employee Leave Request report in a timely manner, and the dates of his doctor's notes show they were issued several weeks after his days of sick time.
